pain in ball of foot

My husband stepped on a sea urchin while on holiday in Greece. A month layer and we have just got the last spine out. However the ball of his foot is sore and feels "bruised". It is very painful to touch. We wonder if it is to do with the injury or is it something completely unrelated like gout?
