Suggestions for Plantar Fasciitis?

After 6 months of self care for plantar fasciitis in my right foot, I have seen a podiatrist and been given custom orthotics We have adjusted them once already due to persistent pain, and I am still having trouble with the "new" orthotics. After wearing the orthotics for a few weeks, new pain has begun in the right metatarsal area, stinging pain in my arch and persistent pain in the heel. After a period of rest, pain with standing and walking is significant. I am stretching regurlarly, rolling a can under my foot, wearing supportive footwear and avoiding walking as much as possible. I am a teacher, so my job does involve a significant amount of standing.

Any ideas on relief? I haven't seen any positive posts regarding injections. What about a cast? Has anyone else had an increase in pain with the orthotics? Is there some kind of "breaking in" period with the orthotics?
